ALL FAMILIES, both new to Jeffco and returning students, begin by logging on to EnrollJeffco. 

• If you are new to Jeffco, you will set up a username and password.  After your initial set-up, you will receive an email confirmation from Jeffco with direction on how to continue. 
• If you are registering a returning Jeffco student, you will use your existing username and password to continue to process. 
• If you cannot remember your user name and/or password, you will need to go https://pam.jeffco.k12.co.us/ to retrieve your information. 

HOW TO CHOICE ENROLL INTO EVERGREEN MIDDLE SCHOOL-

Round 1 Opens 12/05/2023 - Closes January 12, 2024

Families who wish to enroll their student in a school other than their assigned neighborhood school must complete a Choice Enrollment Application from the link below. Choice Students are accepted on a space available basis. Round one applications are prioritized through a lottery process. If needed, waiting lists are prioritized as first in, first offered. A separate application must be completed and submitted for each child. If a student is not accepted at a school during the first round Choice Enrollment, the student will remain on the prioritized wait list and be carried over to Second Round Choice Enrollment.  

Choice enrolled students moving from elementary to middle school and middle to high school must submit a new Choice Enrollment Application for each school.

The following records are required for Choice enrollment, once

• Birth certificate or a copy of the student's passport to verify age and full and complete legal name 
• Current Immunization Record 
• Copy of Transcript 
• Copy of IEP or 504 if applicable, (We reserve a 48 hour period to review IEP records to assure that EMS can provide services.

Step 1 – Log on to EnrollJeffco to begin.

Step 2 - Verify to see if your address is within the Evergreen Middle School articulation area. (If not, you will be applying for Choice Enrollment to Evergreen Middle School).  The Maps and Boundaries information allows you to determine if you are within Evergreen Middle School's boundary area.  

Proof of residency within our boundary will be required if you indicate your residence is within our articulation area.  Acceptable forms of proof are; Bills (Xcel Energy, Comcast, Water, Trash), Jefferson County Colorado Vehicle Registration or a driver's license with the address clearly indicating within the Evergreen Middle School articulation area. 

Step 3– Finalize Enrollment 

*Submit ALL documents to EMS, via email to [email protected] or by fax at 303.982.5021 or in person. Incomplete packets cannot be processed. Call 303-982-5014 with any questions. 

Withdrawal from EMS

Students who leave Evergreen Middle School during the school year, that are NOT transferring to another Jeffco school, will need to complete a withdrawal form, which can be obtained from the Business Manager (Carol Miner, [email protected]). This form must be completed and have all required signatures. The family is also responsible for returning the school-issued Chromebook and charger in good working order and clean, returning all books and paying all fines or outstanding fees owed to the school. All cumulative file documents will be sent to the student’s new school upon receipt of receiving the official Records Request from the new school. 

Transferring from one Jeffco school to another after September 1st

This requires an administrative transfer.  An administrative transfer is an agreement between the principal from the departing school and the principal from the receiving school. It is solely their decision based on a number of variables from each school.  This is now done through EnrollJeffco as well. There is no guarantee the transfer will be granted.
